Agreement to Participate & Liability Waiver
Assumption of Risk:
I understand that participation in fitness training involves inherent risks, including but not limited to musculoskeletal injuries, cardiovascular incidents, serious injuries to bones, joints, muscles, or other bodily systems, and in rare cases, permanent disability or death. I voluntarily assume all risks associated with participation.
Medical Clearance:
I certify that I am in good health and have obtained any necessary clearance from a licensed physician to participate. I will disclose any pre-existing conditions or limitations to my trainer before participation.
Emergency Medical Care:
I consent to first aid and emergency treatment as deemed necessary. I authorize KemboFitness to arrange transportation to a hospital or emergency care center if required, and I accept responsibility for all related costs.
Medical Insurance:
I understand that I am solely responsible for maintaining my own medical insurance and covering any expenses not included in my coverage.
Release of Liability:
In consideration of being allowed to participate, I release and discharge KemboFitness LLC, its owners, trainers, employees, and affiliates from all liability, claims, demands, or causes of action arising from any injury, loss, or damage, whether caused by negligence or otherwise.
Agreement to Follow Instructions:
I agree to follow all safety instructions provided by KemboFitness staff and adhere to facility policies to minimize risk of injury.
